It is suspected the artificial horizon failed as the aircraft was flying in cloud just prior to the crash. The crew of a naval launch witnessed the crash and searched the area but all they could find was a mae west life jacket, FS. R. Reynolds and three airmen, not Canadians, were also killed. P/O. Ives had gone missing on August 19, 1941 during a bombing operation with #51 Squadron but turned up safely at Gibraltar on November 18, 1941.
